Deposit Form

The Deposit Form is used to submit funds to the PTA for deposit into the PTA bank account.  All funds collected should be submitted with the Deposit Form within 5 business days of the event date or sale close.  Additional documentation to explain the funds collected may also be required.  Deposit Forms and funds should be submitted to the PTA Financial Assistant. Committee Chairs are required to adhere to the Money Handling and Deposit Procedures.
